PROCESS

FIRST TIME applicants:

STEP 1: Get your Order of Payment and pay Php194.00 for your Health Card at the City Treasurer's cashier

STEP 2: Submit for laboratory examination your stool and sputum specimen at Window 1&2.The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories (please see listing).

STEP 3: Get the results at Window 3.

STEP 4: FOR NEW APPLICATION: Attend an HIV/AIDS seminar. Pick a schedule in any of the 5 sessions everyday every: 9:00 AM, 10:00 AM, 11:00 AM, 1:30 PM, 3:00 PM.

STEP 5: Get your picture taken at the Health Department for your Health Card

STEP 6: Get your Health Card

RENEWAL of Health Permit

STEP 1: Get your Order of Payment and pay Php144.00 for your Health Card at the City Treasurer's cashier.

STEP 2: Submit for laboratory examination, your stool and sputum specimen and get the results. The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories (please see listing)

STEP 3: Get the results at Window 3.

STEP 4: Proceed to Receiving Area to claim your new health certificate

 

REQUIREMENTS 

Fill up an application form and submit to the Sanitation Division Office the following requirements:
a.) Official receipt of payment of the Sanitary Fee

b.) Inspection report of the Sanitation Inspector to include the following among others:
* Microbiological Analysis of Water- from water laboratory accredited by the QC Health Department
* Pest Control Contract of Service / Agreement of Contract Service from licensed pest applicator
* Masterlist of all employees of the establishment with respective designation/ position

c.) Original Health Certificate of all employees of the establishment including that of the Manager(s) and/or Owner for presentation.

d.) Photocopy of the Health Certificates-for submission

e.) Schematic Diagram / Floor Plan of the establishment

f.) Occupancy Permit of the establishment

g.) Photocopy of previous Sanitary Permit (for renewal)

ADDITIONAL REQUIREMENTS FOR:
a.) Applicants below 18 years old:
* Parental consent
* Birth certificate/baptismal certificate
* Valid ID of the parent/guardian giving consent

b.) Massagists/ Therapist/Massage Clinic Attendants/Reflexologists, etc:
* Massagists course
* Original Birth Certificate issued by NSO
* Marriage Certificate (if married)
* 1 pc. 2x2 w/ white background
* pcs. 1x1 w/ collar & sleeves
* Lab. Result : VD Clearance
* Official Receipt (P 309.00)
* Massagists’ Training Certificate
* Date of training: Every 3rd wk of the month Tuesday to Friday; 1-5PM

c.) GRO/FRM/DANCER/ENTERTAINER, etc.:
* Original Birth Certificate from NSO/ Local Civil Registry
* Marriage Certificate (if married)
* 1x1 ID picture
* Lab. Result : VD Clearance
* Official Receipt (P 259.00)

SPECIAL SERVICE (Companies can apply for the Health Certificate of their employees)
a.) Requirements:
* Authorization letter
* Photocopy of the ID of both requester and company’s representative.
* Submit all the requirements for health certificates stated above.
* The company can address letter of authority to:

b.) Companies can request to:
* Have their employees' stool and sputum examination done at any accredited laboratories (please see listing).
* Have their HIV/AIDS seminar done for their employees at their company premises.
* The City Health Department will send the lecturer(s) at the agreed upon schedule.
* Submit their employees' photos for their Health Certificate. If the photo-taking is not done at the Health Department, the ID provided will be a "white" Health Certificate, and not a self-laminated card. This “White” Health Certificate is good only for one year, while the self-laminated Health Certificate has provisions for 3 years use.
