PROCESS
 
STEP 1: Go to the Miscellaneous Section of the City Treasury Office 
 
STEP 2: Proceed at Window 12 then present all applicable requirements 
 
 
REQUIREMENTS
 
For NEW Application:
a.) PRD ID
b.) Authorization letter (if representative will appear on behalf of the applicant)
 
For RENEWAL
a.) Original previous receipt
b.) PRD ID
c.) Authorization letter (if representative will appear on behalf of the applicant)
 
 
FEE
 
A fee of Php300 will be charged every month of January (renewal month) whether application is new or renewal. A penalty of Php75 plus 2% (Php82.50) will be added if payment will be made on the month of February. x2% for the succeeding months (March-December).
