Paalala sa mga tatanggap ng UCT Cash Card na dumating sa takdang oras, petsa, at venue na naka-assign sa inyong barangay. Dalhin din ang mga sumusunod na requirements:

A. For Beneficiaries

1. One (1) original and photocopy of OSCA ID

B. For Authorized Representative

1. Original and photocopy of OSCA ID
2. Authorization letter
3. Original and photocopy government issued valid ID of representative
4. Picture of beneficiary together with authorized representative holding a newspaper or calendar of January 2023

C. For Deceased Beneficiaries

1. One (1) original and photocopy of OSCA ID
2. PSA of CTC LCR Death Certificate of beneficiary
3. Special Power of Attorney
4. Proof of relation of representative
5. Affidavit of kinship (representative who are not spouse or child)
6. Warranty and release from liability (provided by DSWD)
