Para sa mga kwalipikadong senior QCitizens ng District 6, magtungo lamang sa bagong District 6 Action Center sa SB Park, Neptune St., Brgy. Tandang Sora (near Circle C Congressional).

A. Senior Citizen
1. Iprisinta ang inyong Quezon City ID
2. Photocopy ng Quezon City ID na may 3 pirma
B. Authorized Representative
1. Iprisinta ang Quezon City ID ng senior citizen
2. Photocopy ng Quezon City ID na may 3 pirma o thumbmark ng senior citizen
3. Authorization letter na nilagdaan ng benepisyaryo at ng representative
4. Magprisinta ng government-issued ID ang representative
5. Photocopy ng government-issued ID ng representative
