Booking an Appointment
- Patient or Relative to submit patient’s personal details to the Quezon City Health Department Telemedicine Coordinator:
- Name
- Contact Number
- Email Address
- Preferred manner of teleconsult – video call, voice call or SMS
- Send details through the following:
- Personal delivery to the Health Centers
- Social Media
Teleconsult
- Patient or Relative will receive a notice of video/audio/sms consultation schedule.
- For patients preferring video call, they will have to click the link provided to consult with the physician.
- For patients/relative choosing audio call or SMS, the physician will get in touch with patient/relative.
