Booking an Appointment

  1. Patient or Relative to submit patient’s personal details to the Quezon City Health Department Telemedicine Coordinator:
    • Name
    • Contact Number
    • Email Address
    • Preferred manner of teleconsult – video call, voice call or SMS
  2. Send details through the following:


  1. Patient or Relative will receive a notice of video/audio/sms consultation schedule.
  2. For patients preferring video call,  they will have to click the link provided to  consult with the physician.
  3. For patients/relative choosing audio call or SMS, the physician will get in touch with patient/relative.
