GENERAL REQUIREMENTS:

• Business Permit for the present year from the Business Permits and Licensing Department 
• Business Permit Official Receipt 
• Employee Health Certificates from the Quezon City Health Department 
• Health Occupancy Permit Receipt (for New Applicants and Newly Renovated Establishments) 

ADDITIONAL REQUIREMENT FOR EXISTING BUSINESSES WITHOUT PREVIOUS RECORDS OF SANITARY PERMIT:

• Affidavit of Undertaking 

ADDITIONAL REQUIREMENTS FOR BUSINESSES PROVIDING FOOD AND WATER:

• Microbiological Water Analysis from a DOH-accredited lab (Every Other Month)
• Pest Control Service Report from FDA-accredited service provider (Monthly) 

REQUIREMENTS FOR NON-FOOD BUSINESSES:

• License to Operate from the Department of Health (Original Copy) 
• Microbiological Water Analysis from a DOH-accredited lab (Twice a Year) 
• Pest Control Service Report (Twice a Year) 
• Physicochemical Water Analysis Report (Every 6 Months)

NOTE: Laundry shops, pest control operators, and lessors of rooms, houses, and apartments are exempt from submitting a water analysis and pest control service report. 

APPLICATION PROCESS:

1. Log in to QC eServices and select Health Certificate & Sanitary Permit, which can be found under Clearances, Permits, and Zoning.
2. Tap the Sanitary Permit icon, then indicate the application type.
3. Fill out the required information and upload all documentary requirements based on the nature of the business.
4. Pay the PHP 100.00 inspection fee via online fund transfer unless the fee is already included in your current business tax compliance.
5. Wait for the final approval of your application.
6. Claim your sanitary permit from the Quezon City Health Department Sanitary Division located on the 2nd floor of the QC Health Building at Quezon City Hall, Mayaman St. corner Kalayaan Ave. Gate 4.

NOTE: New businesses will receive a digital copy of their provisional sanitary permit.
