Minors (1-17 years old) may register or get registered for a QCitizen ID. Minors aged 15-17 can obtain a physical QCitizen ID card, while those aged 1-14 will only receive a digital QCitizen ID in the QCitizen App. After legally becoming an adult, the proof of residency, photo, and other QCID requirements, whether for residents or non-residents, must be updated.

GENERAL REQUIREMENTS:

• Proof of Identity (PSA Birth Certificate or LCR Certificate of Live Birth)
• School ID (Optional)
• Proof of Residency:
  • If Living with Parent:
    • QCID of Parent
• If living with a guardian:
  • QCID of the Legal Guardian
  • Affidavit of Guardianship

• If living alone:
  • Notarized Lease Contract (with signature of lessor and parent/legal Guardian, and states that the minor is an authorized occupant)
  • Photocopy of Valid ID of all signatories
  • 2 Billing Statements (most recent & one six months prior)

• PWD Minors
  • Valid ID or Original Birth Certificate of Residency that proves you are a Quezon City resident

REQUIREMENTS FOR PWD:

• For apparent disability: Bring a whole-body photo showing the disability. No need for a birth certificate.

• For non-apparent disability: Certificate of Disability from a Specialist or Health Office

STEPS:

1. Register or log in to QC E-Services.
2. Choose and click ‘QCitizen ID eApplication.’
3. Step-by-step instructions will be shown for your guidance.

4. A notification will be sent upon the approval of your application. Should your application be rejected, you will be informed of the reason for rejection. Simply reapply and submit the corrected data/document.
5. For ages 15-17, you may have your QC ID printed from the district action office near you if your ID application status indicates ‘FOR CARD PRINTING’ or you may claim it at the main QCID office located on the 2nd floor of the High-Rise Building at Quezon City Hall once the status changes to ‘CARD PRINTED.’
