The milk donated to the Quezon City Human Milk Bank will be given to babies in critical condition, such as preterm babies or those affected by calamities.
AVAILING HUMAN MILK
REQUIREMENTS:
- QCitizen ID
- Clinical abstract from the hospital where the baby was admitted
- Prescription from the baby’s pediatrician
- Icebox or cooler with gel pack
- Two (2) physical valid government IDs of the baby’s mother (the home addresses must be the same)
- Government Employee ID or Persons with Disability (PWD) of the baby’s parent to avail discount (if applicable)
- Authorization from the mother (if authorized representative)
- One (1) physical valid government ID of representative (if authorized representative)
PROCESS:
- Call or inquire from QCHMB, which is located on the 2nd floor of Quezon City General Hospital, Seminary Road, Brgy. Bahay Toro, Quezon City.
- Show the requirements.
- Sign the consent and waiver form for availing pasteurized human milk.
- Pay the fees (processing fee: Php 220 per 100 ML; bottle fee: Php 280 per milk bottle) at the cashier station, or you may head directly to the Medical Social Service to receive the discount or “financial assistance” based on the classification.
NOTE: The Quezon City Human Milk Bank is open daily, from 8:00 AM to 5:00 PM, and during holidays from 8:00 AM to 12:00 NN.
DONATING HUMAN MILK
ELIGIBILITY:
- Does not smoke or use nicotine products
- Does not regularly consume alcohol
- Does not use illegal/recreational drugs
- Does not take certain medications or herbal supplements
- Do not have infectious diseases like HIV, Hepatitis B or C, HTLV, or syphilis
- No history of cancer, heart disease, high blood pressure, or a major illness/surgery within the last 12 months
- Did not receive a blood transfusion or organ/tissue transplant recently
- Does not have a tattoo for less than 12 months/1 year
WALK-IN PROCESS:
- Visit the Quezon City Human Milk Bank located on the 2nd floor of the Quezon City General Hospital.
- Express your interest in donating breast milk and wait for the milk collection.
- Undergo a thorough screening, encompassing an interview, a free physical examination, and a free serological test to check for HIV, Hepatitis B, and Syphilis before you can donate.
MILK PICK-UP PROCESS:
- Call or send a message to any of the contacts below for the schedule of the milk pickup in your area.
- Wait for confirmation regarding the milk collection.
- Undergo a thorough screening, encompassing an interview, a free physical examination, and a free serological test to check for HIV, Hepatitis B, and Syphilis. You may also send or show your test results dated within the last 6 months.
- Milk will be collected with the use of breast pumps.
MILK LETTING ACTIVITY PROCESS:
- Follow the Facebook page of the Quezon City Human Milk Bank or visit the website to stay updated on the schedule of milk-letting activities at the health centers and lying-in clinics near you.
- Undergo a thorough screening, encompassing an interview, a free physical examination, and a free serological test to check for HIV, Hepatitis B, and Syphilis.
- Milk will be collected with the use of breast pumps.
