ONLINE REGISTRATION & BOOKING

STEP 1:       
IF YOU HAVE A QCeServices Account
Go to qceservices.quezoncity.gov.ph and log in to your QC Eservices Account and Click on “QC Vax Easy” from the list of services

IF YOU DO NOT HAVE A QCeServices Account
Go to qceservices.quezoncity.gov.ph/qcvaxeasy and click “Manage Your Vax Easy Registration”

STEP 2:
Click “Book Vaccination”

STEP 3:
Click on the Calendar Button/Icon.  The Calendar will pop up. Choose your preferred vaccination date.

STEP 4:
Choose your preferred vaccination site from the list of your sites for your chosen date by clicking on the circle icon beside the name of the Vaccination Site.

STEP 5:
Under Time, click on the Calendar Button/Icon and choose your preferred time. Click ‘Submit’ to submit your preferred vaccination schedule.

STEP 6:
Click ‘Submit’ to confirm your vaccination appointment, Do not close your window until you see a message that your vaccination has been successfully booked.  Please take note of this information

BARANGAY ASSISTED BOOKING

STEP 1:
Get in touch with your barangay. You can check the
Barangay directory here: https//quezoncity.gov.ph/quezon-city-barangay-officials

STEP 2:
Fill-up the Assisted Booking Form to be given, and receive the Confirmation Slip

STEP 3:
Arrive 15 minutes before your appointment schedule at the vaccination site.

STEP 4:
Do not forget to bring your important documents on the day of your vaccination schedule
