Under the SMART Child e-Habilin program, the children and families left behind by OFWs can be regularly monitored. Children and their guardians may participate in focus group discussions and psychosocial workshops, as well as engage in weekly skills development activities.
REQUIREMENTS:
- QC ID
- Any Government Issued ID of the OFW with signature
- Passport Information Page and Last Departure Date Stamp Page
- Overseas Employment Contract Certificate (OEC) or any proof of overseas employment
- Proof of relationship with the child (Both Guardian and Parent)
- Birth certificate of the child
- Consent Letter with Signature of the OFW authorizing the Guardian to Apply the child for Smart Child e-Habilin Program
PROCESS:
- Visit QC E-services and register or log in with your email address or Facebook account.
- Scroll down the list of services and click SMART Child e-Habilin.
- Click ‘Submit New Application.’
- Fill out the sections for OFW Parent/s Information and the general information of the guardian and child.
- Upload the requirements. File type must be either .pdf, .jpg, .jpeg, or .png.
- Check the box indicating you are giving your consent to the Quezon City Government (QCG) to monitor and provide necessary support to your child.
- Check the box to certify that all information given is correct.
- Click ‘Submit.’
