STEPS VIA QUEZON CITY GOVERNMENT WEBSITE:

1. Look for the People’s Corner section on the homepage of Quezon City Government website and  click, ‘Request for Information.’
2. After reading the disclaimer, you will be redirected to QC E-Services, where you may log in using an email or a Google or Facebook account. 
3. Read the ‘Exceptions to Right of access to Information’ to know whether the information you need is open to the public.
4. Select the department, agency, or office involved. 
5. Indicate the information, document, or record that you need.
6. Explain the purpose of your request.
7. Click your preferred type of copy: digital (Free) or physical (Additional charges may apply).
8. Read the Privacy Notice regarding the use of information and check the box indicating that you have read and understood its content.
9. Click ‘Submit.’

STEPS VIA QC E-SERVICES:

2. Scroll down the list of services on the dashboard and click ‘Request Information.’
3. Read the disclaimer regarding information accuracy and data usage and check the box indicating that you have read and understood its content.
4. Read the ‘Exceptions to Right of access to Information’ to know whether the information you need is open to the public.
5. Select the department, agency, or office involved. 
6. Indicate the information, document, or record that you need.
7. Explain the purpose of your request.
8. Click your preferred type of copy: digital (Free) or physical (Additional charges may apply).
9. Read the Privacy Notice regarding the use of information and check the box indicating that you have read and understood its content.
10. Click ‘Submit.’
