REQUIREMENTS:

• Barangay Indigency (Purpose: For Social Welfare Assistance)
• Medical Certificate 
• QC Person with Disability ID (if without a medical certificate)

PROCESS:

1. Go to QC E-Services and log in to your QC E-Services account.
2. Click the program “QC Social Welfare Assistance,” which can be seen on the Dashboard.
3. Provide the following details: Personal Information, Occupation, Education Background, Additional Information, Family Composition, Breakdown of Monthly Expenses.
4. Upload the required documents and make sure that you attached the correct files.
5. After completing the Online Application Form, check the details carefully and click “Submit”.
6. Select a district action office from among the branches and schedule an appointment.
7. Monitor your online application.
