Issuance of Health Certificate

The Health Certificate is issued to all persons involved in the operation and management of an establishment regardless of the job description, upon compliance with all the requirements set by the Quezon City Health Department.

The Sanitation Inspector or any HIV Seminar trained personnel of the QCHD assigned in the area will conduct the said seminar orientation at an appropriate venue or site on an agreed schedule where physical distancing is applicable.

Issuance of Yellow Health Certificate

The Yellow Health Certificate or Card is issued to a person employed as masseur, masseuse, massage therapist, massage attendant, sauna bath attendant, guest relation officer, sexy dancer, escort, model of nightclubs, and other related practitioners but not limited to managers and other non-professional entertainers, upon compliance with all the requirements set by the Quezon City Health Department.

Issuance Of Sanitary Permit (SP)

The Sanitary Permit is a written authorization or certification issued by the City Health Officer which signifies the establishment’s compliance with the existing requirements upon inspection or evaluation by the Sanitation Inspector or Evaluator.

Issuance of Provisional Sanitary Permit for New Businesses

The Provisional Sanitary Permit is a temporary certification in writing issued by the City Health Officer to the New establishment which may partially initiate business activity and shall be given ample time in complying with the existing sanitary requirements upon evaluation or inspection by the Sanitation Inspector. This authorization remains valid until the indicated validity period.

Issuance of Pre-Marriage Counseling Certificate

The Pre-Marriage Counseling Certificate is a written documentation issued by an accredited counselor upon completion of documentary requirements and attendance to the counseling session prior to actual marriage. Pursuant to Article 16 of the Family Code, this mandatory pre-marriage counseling session is required to all contracting parties which provides assistance and guidance to both parties towards an informed decision about their forthcoming married life.

Maternal & Child Care Program

This program provides universal access to its services for children from birth to school age and their families. According to WHO, Maternal and Child Health services can be defined as promoting, preventing therapeutic or rehabilitation facilities or care for the mother and child. It is also an important and essential service related to mother and child overall development.

A. Pre-Natal care

    How to Avail of Free Medicines

    How to Avail of Free Medicines

    The program provides free medicines for fever, cough and colds; medicines for the prevention of tuberculosis; and other medicines like antibiotics and vitamins for Quezon City residents.  Each Quezon City Health Center has a medicine dispensing room where residents may avail of the medicines with a doctor’s prescription. The program also provides free medicines for indigent QCitizens diagnosed with high blood pressure, hypertension, diabetes and high cholesterol.

    REQUIREMENTS WHEN CLAIMING FROM HEALTH CENTERS:

    1. Prescription from the Health Center’s licensed doctor.
    2. If the doctor’s prescription has expired, the person may consult with the Health Center doctor to get a new medical prescription. The person may present another doctor’s prescription.
    3. QCitizen ID or any valid ID with a Quezon City address.

    REQUIREMENTS WHEN CLAIMING FROM DISTRICT ACTION OFFICES:

    1. QCitizen ID or any valid ID with Quezon City address or Certificate of Indigency.
    2. Licensed medical doctor’s prescription.

    PROCESS FOR CLAIMING FREE MEDICINES:

    1. Go to the nearest Quezon City Health Center or District Action Office.
    2. Present the doctor’s prescription or seek medical consultation at the Health Center to secure a medicine prescription.
    3. Bring the prescription and give it to the dispensing officer to log it in the dispensing record.
    4. Once the patient receives the medicines, the dispensing officer will ask the patient to sign in  the dispensing logbook.
    5. If there is no available supply of the said medicine, the dispensing officer will refer the patient or client to the Social Services Development Department with a proper referral slip that indicates the complete  details of the patient.

    PROCESS FOR CLAIMING MAINTENANCE MEDICINE:

    1.If you are already a regular patient or client of the Non – Communicable disease program, you can show your prescription directly to the medicine dispensing officer to get your medicine that was issued for the last 3 months..

    2.Patients with hypertension, high cholesterol, or diabetes may have their consultation at the nearest Health Center and be registered to the Non – Communicable Disease Program.

    3 Go to your District Action Office to get your initial free medicine (good for one week). Proceed to the Health  Center for check-up  and get registered with the Non- Communicable Disease Program.

    REMINDERS:

    1. For antibiotic medicine,  the prescription  is valid  for one week’s supply only. For vitamins and maintenance medicines , the prescription is valid for three months.
    2. For an old prescription, visit the nearest Health Center for consultation to secure your new medicine prescription.
    3. For Bedridden Patients, an authorized person is assigned to deliver the medicine supply at home. If not, then relatives or an authorized member of the family is allowed to get the medicines at the Health Center. Kindly bring one photocopy each of a valid ID of the patient as well as the ID of the authorized person.
    4. Only generic brand of medicines are available at the Health Centers and District Action Offices.

    All registered patients in the Non- Communicable Disease Program  must have a quarterly follow -up check -up  every three ( 3) months and seek consultation when needed.

    Molecular Diagnostics Laboratory

    The Molecular Diagnostic Laboratory processes the samples from the coronavirus disease COVID-19 community testing center. The laboratory can process 500 to 1000 tests per day with only one to two day turnaround time. The laboratory can process not only COVID-19 tests but also other medical test like tuberculosis , human immunodeficiency virus (HIV) and dengue.

    Social Hygiene Clinics are responsible for the management, prevention, and control of sexually transmitted diseases (STD). The clinic’s services include providing diagnostic assessment, treatment, and care; promoting health and preventing STD and skin diseases through individual counseling and other health education activities; contact tracing for investigation and defaulters for treatment and follow-up. The clinic encourages male and female patients to avail of its services such as free testing and screening, treatment, free HIV counseling and other support services.
