  • Related provisions of the 1987 Philippine Constitution which authorized the state to provide comprehensive and accessible health care programs.
  • Republic Act No. 7160, otherwise known as the Local Government Code of 1991, directed the devolution of health care service delivery to local government units.

ISSUANCE OF HEALTH CERTIFICATE

The Health Certificate is issued to all persons involved in the operation and management of an establishment regardless of job description, upon compliance with all the requirements set by the Quezon City Health Department. 

  • Issuance of Health Certificate Clinical Laboratory Services for New Applicants who will avail of the laboratory services from any QCHD  CLINICAL / IN-HOUSE LABORATORY through the Quezon City  Online Registration & Application. 
  • Issuance of Health Certificate Clinical Laboratory Services for Renewal Applicants who will avail of the laboratory services from any QCHD CLINICAL/IN-HOUSE LABORATORY through the Quezon City Online Registration & Application.
  •  Issuance of Health Certificate Recognized Laboratory Services for New Applicants who will avail of the laboratory services from on EXTERNAL LABORATORY  via Walk-In Application. 
  • Issuance of Health Certificate Recognized Laboratory Services for New Applicants who will avail of the laboratory services from an EXTERNAL LABORATORY  through the Quezon City Online  Registration & Application
  • Issuance of Health Certificate  Recognized Laboratory Services for Renewal Applicants who will avail of the laboratory services from an EXTERNAL LABORATORY via Walk-in Application. 
  • Issuance of Health Certificate New / Renewal Bulk Applicants.

Bulk application involves thirty (30)  applicants and above, within the same establishment or with the same owner or management, upon submission of a request letter for the conduct of HIV Seminar from the establishment administrator, manager or representative, addressed to the  City Health  Officer. The Sanitation  Inspector or any HIV Seminar trained personnel of the QCHD assigned in the area will conduct the said seminar orientation at an appropriate venue or site on an agreed schedule where physical distancing is applicable.

ISSUANCE OF YELLOW HEALTH CERTIFICATE

The Yellow Health Certificate or Card is issued to a person employed as masseur, masseuse, massage therapist, massage attendant, sauna bath attendant, guest relation officer, sexy dancer, escort, model of nightclubs, and other related practitioners but not limited to managers and other non-professional entertainers, upon compliance with all the requirements set by the Quezon City Health Department.

The  Sanitary Permit is a written authorization or certification issued by the City Health Officer which signifies the establishment’s compliance with the existing requirements upon inspection or evaluation by the Sanitation Inspector or Evaluator.

ISSUANCE OF PROVISIONAL SANITARY PERMIT FOR NEW BUSINESSES

The Provisional Sanitary Permit is a temporary certification issued by the City Health Officer via the online unified business permit application system to new establishments which may partially initiate business activity given ample time in complying with the existing sanitary permit requirements based on the nature of business. This authorization remains valid until the indicated validity period.

ISSUANCE OF PRE-MARRIAGE COUNSELLING CERTIFICATE

The Pre-Marriage Counseling Certificate is a written document issued by an accredited counselor upon completion of documentary requirements session prior to actual marriage. Pursuant to Article 16 of the Family Code, this mandatory pre-marriage counseling session is required to all contacting parties which provides assistance and guidance to both parties towards an informed decision about their forthcoming married life.

MATERNAL & CHILD CARE PROGRAM

This program provides universal access to its services for children from birth to school age and their families. According to WHO, Maternal and Child Health services can be defined as promoting, preventing therapeutic or rehabilitation facilities of care for the mother and child. It is also an important and essential service related to the mother and child’s overall development.

A. Pre-Natal Care

          1. Check-ups and consultation

          2. Simple laboratory ( urine, FBS, Hemoglobin, Hematocrit, HIV Testing)

          3. Tetanus Toxoid immunization

          4. Provision of iron tablets and low dose of vitamin A.

          5. Referral of high-risk cases for secondary or tertiary care

          7. Promotion of exclusive breastfeeding

          8. HIV counseling and testing of pregnant women

B. Natal Care

  1. Delivery at (9) lying-in clinics

NATIONAL IMMUNIZATION PROGRAM

One of the pioneer programs of the Department of Health, established in 1976, which aims to provide immunity against vaccine-preventable diseases. Pres. Marcos Mandated Immunization PD No. 996.

September 16, 1976, provided for the compulsory basic immunization for infants and children below 8 years old recognizing that they are the most important assets of the nation. Thus, every effort must be exerted to promote their welfare and enhance the opportunities to lead a useful and happy life.

NON-COMMUNICABLE DISEASE CONTROL PROGRAM

This program focuses on the prevention and control of cancer, diabetes, cardiovascular conditions, and stroke. The program promotes reducing risk factors associated with these diseases including, detecting, screening, and treatment.

Integrated Non-Communicable Disease Prevention:

  1. Hypertension/Diabetes cases
  2. HD Club
  3. Free medication for Hypertension/Diabetes
  4. Free Cholesterol screening
  5. Proper nutrition counseling
  6. Weight loss program
  7. Health Education through flyers, tarpaulin, and social media.

NUTRITION PROGRAM

The role of nutrition and health in economic and social development cannot be undermined. According to the World Health Organization (WHO), health is not the only absence of disease but a state of complete mental and physical well-being in relation to the productivity and performance of an individual. Good nutritional status plays important in realizing food security within the families and communities. Food and Agriculture Organization define food security as access by all people at all times to the guaranteed commitment and right of state, every Filipino has the right to adequate nutritious food. The objective is to promote positive nutritional behavior through holistic nutrition services to empowered QCitizens of all life stages.

On Nutrition Program:

  1. General Operation Timbang plus mass weighing of (0-70 mos.) old children to identify malnourished children.
  2. Complementary feeding Program for 6-24-month-old identified underweight, severely underweight, wasted, and severely wasted.
  3. Malnutrition rehabilitation (Ready to use therapeutic food for severe acute malnutrition).
  4. Harmonized Deworming since intestinal parasitism contributes to malnutrition.
  5. Iron supplementation prevalence of Anemia.
  6. Nutrition advocacy (on breastfeeding, food fortification, and integrated helminthiasis and control program).
  7. Implementation on Milk Code including Breastfeeding and Beyond.
  8. Social Mobilization on Milk Letting Activities.
  9. Establishment of Breastfeeding in the Workplace
  10. Breastfeeding support groups 
  11. Technical assistance on Mother-Baby Friendly Hospital initiatives Accreditation.

NATIONAL TUBERCULOSIS PROGRAM

  1. Screens Presumptive Drug susceptible TB and Drug Resistant ( TB Adult and Children )
  2. Performs free diagnostic services sputum examination, XPERT
  3. Tuberculin Testing in Children
  4. Performs other laboratory tests: HIV Testing and blood sugar tests.
  5. Refer sputum /XPERT negative but X-ray positive to TB diagnostic.
  6. Committee to review and recommendation of management.
  7. Provide medical consultation
  8. Counseling on lifestyle modification like smoking, alcohol, and substance abuse.
  9. Provides TB medicines ( Drug Susceptible TB and Drug resistance TB)

CANCER CONTROL PROGRAM

  1. Visual Inspection  using acetic acid for cervical cancer (Pap smear)

   2. “Salat Suso” for Breast Cancer 

   3. Digital Rectal Examination for Prostate Cancer

   4. Health Education

   5. No smoking campaign for prevention of lung cancer

  3. Refer Men and Women BTL, Non-Scalpel Vasectomy
  4. Management of side effects due to use of Family Planning Method

ADOLESCENT PROGRAM

  1. General Assessment, History, and Physical Examination

  9. Home Care and follow-up visits to bedridden
  10. Assist elders in OSCA and PhilHealth enrolment
  11. Provision of mobility devices (wheelchair, walker, cane) to disabled elderly patients

EPIDEMIOLOGY AND SURVEILLANCE PROGRAM

MEDICAL MORBIDITY CLINIC (GENERAL CONSULTATION)

  1. Free Consultation and Management of patients/clients at the Health Centers, mobile, and during outreach Clinics
  2. Provision of free medicines for treatment of cases
  3. Referral for Laboratory exams and for further management in hospitals
  4. Counseling and information circumstances
  5. Outreach clinics in special circumstances activities and outbreaks

HEALTH EMERGENCY MANAGEMENT PROGRAM

  1. The Health Emergency Management Program is a structured approach used by healthcare organizations, governments, and other relevant stakeholders to prepare for, respond to, and recover from health emergencies and disasters. It seeks to safeguard public health and safety by ensuring that healthcare systems and communities are adequately equipped to promptly respond to and recuperate from emergencies and catastrophes. The effectiveness of these programs relies on the crucial collaboration among government agencies, healthcare organizations, community groups, and other stakeholders.
  2. Key components include prevention and mitigation, preparedness, response, and rehabilitation and recovery. Prevention and mitigation involves identifying vulnerabilities and risks, while preparedness involves planning, training, and resource allocation. The response involves activating emergency plans, coordinating resources, and communicating with the public. Recovery focuses on restoring normal operations, rebuilding infrastructure, and addressing the physical, emotional, and economic impacts of the emergency.

HOW TO GET A FREE MEDICINES

Requirements:

  • QC ID or any valid with Quezon City address or certificate of indigency.
  • Licensed medical doctor’s prescription.

Steps: 

  1. Go to the nearest Quezon City Health Center or District Action Office.
  2. Present the doctor’s prescription or seek medical consultation at the Health Center to secure a medicine prescription.
  3. Bring the prescription and give it to the dispensing officer to log it in the dispensing record.
  4. Once the patient receives the medicines, the dispensing officer will ask the patient to sign in the dispensing logbook.
  5. If there is no available supply of the said medicine, the dispensing officer will refer the patient or client to the Social Services Development Department with a proper referral slip that indicates the complete details of the patient.

HOW TO GET A MAINTENANCE MEDICINES

Requirements:

  • QC ID or any valid with Quezon City address or certificate of Indigency
  • Licensed medical doctor’s prescription

Steps:

  1. If you are already a regular patient or client of the Non-Communicable Disease program, you can show your prescription Directly to the medicine dispensing officer to get your medicine that was issued for the last 3 months.
  2. Patients with Hypertension, high cholesterol, or diabetes may have their consultation at the nearest Health Center and be registered to the Non- Non-Communicable Disease Program.
  3. Go to your District Action Office to get your initial free medicine (good for one week), proceed to the Health Center For Check-up, and get registered with the Non-Communicable Disease Program.

JACKSONVILLE MEDICAL MISSION

The Quezon City Government’s Health Department collaborated with the Philippine Medical Society of Northeast Florida Inc. to conduct large-scale medical services in Quezon City from January 22-27, 2024. The said activity provided services to 1000-plus QCitizens each day for the duration of the event.

MOLECULAR DIAGNOSTICS LABORATORY

The Molecular Diagnostic Laboratory processes the samples from the coronavirus disease COVID-19 community testing center. The laboratory can process 500 to 1,000 tests per day with only one to two day turnaround time. The laboratory can process not only COVID-19 tests but also other medical test like Tuberculosis, Human Immunodeficiency Virus (HIV) and dengue.

Lying–in Clinics are maternity clinics for healthy pregnant women as an option for an affordable, if not free, cost of pregnancy and childbirth. If a pregnant woman is at high risk, however, she will be referred to deliver the child at a hospital.

SOCIAL HYGIENE CLINICS

Social Hygiene Clinics are responsible for the management, prevention, and control of sexually transmitted diseases (STDs). The clinic’s services include providing diagnostic assessment, treatment, and care; promoting health and preventing STD and skin diseases through individual counseling and other health education activities; contact tracing for investigation and defaulters for treatment and follow-up. The clinic encourages male and female patients to avail of its services such as free testing and screening, treatment, free HIV counseling, and other support services.

SUNDOWN OR KLINIKA

Sundown/Klinika is a type of Social Hygiene Clinic that provides free consultation, treatment, and counseling services for men who have sex with men (MSM) and those men vulnerable to the human immunodeficiency virus.
