The validity of a health certificate is one (1) year from the date of issuance. If you are a first-time job seeker, please secure and present a certification from the QC Public Employment Service Office to have the health certificate fees waived.

REQUIREMENTS:

• Business permit of nano-enterprise (if applicable) 
• Fecalysis/Stool Exam Result from a DOH-accredited facility 
• Sputum/Chest X-ray Exam Result from a DOH-accredited facility 
• HIV Seminar / Health Card (front and back)
• 2X2 ID Picture with white background

ADDITIONAL REQUIREMENTS FOR UNDERAGE APPLICANTS:

• Original copy of birth certificate
• Written parental/guardian consent
• Original Valid ID of Parent/Guardian giving consent

PROCESS:

1. Log in to QC eServices and look for Clearances, Permits, and Zoning from the list of services.
2. Select Health Certificate & Sanitary Permit, then choose Health Certificate.

3. Indicate whether the purpose of the transaction is for renewal or a new application.
4. Complete the online form and indicate whether you are a PESO beneficiary.
5. You may pay the corresponding fees online, at the Quezon City Treasurer’s Office, or in any of the CTO satellite payment centers.
   • HIV seminar fee: PHP 50
   • Health certificate processing fee: PHP 75
   • In-house sputum exam fee: PHP 110
   • In-house stool exam fee: PHP 65

6. Attend the online HIV seminar and answer the questionnaires (for new applications only).

NOTE: If a valid HIV seminar certificate has already been uploaded, the applicant will skip this step.

7. You may either book an appointment to avail of the QC in-house laboratory services or immediately upload your lab results from a DOH-accredited facility. 

8. Wait for the issuance of your digital health certificate, then print it in the prescribed size (14 cm x 9 cm).
