Description
The Office of the City Mayor (OCM) enforces all laws and ordinances relative to the governance of the city and implements all approved policies, programs, projects, services, and activities of the city; initiates and maximizes the generation of sources and applies the same to the implementation of development plans, programs, objectives; and ensures the delivery of basic services and the provision of adequate facilities for the city.

REQUEST FOR ISSUANCE OF MAYOR’S CLEARANCE AND/OR CERTIFICATION

Issuance of clearance and/or certification from the Office of the Mayor for purposes of the following:

  1. Local employment
  2. Business Requirement
  3. PNP/AFP/PMA/Reservist/BFP Admissions
  4. Travel Abroad
  5. Record check
  6. Marriage requirement
  7. Correction of Entry (Birth Certificate)
  8. License to own firearms/sell firecrackers
  9. Certification to Solemnize Marriage
  10. Municipality Certification of Unemployment (DepEd, SHS Voucher, and ESC)
  11. Authentication and/or Certification of Residency (DFA req)

How To Process Late Registration for Certificate of Live Birth

Free Late registration of Certificate of Live Birth from ages zero to seventeen years old (0 to 17) for indigent parents whose child was born in Quezon City.

  Requirements:

  1. Fully accomplished Certificate of Live Birth Form (Municipal Form No. 102)
  2. Signed Affidavit for Delayed Registration (at the back of the Certificate of Live Birth Form)
  3. Negative Certification of Birth from the Philippine Statistics Authority- (PSA)
  4. Certificate of No Record (CNR) from the Local Civil Registry
  5. 1 photocopy of the Marriage Certificate of Parents (if married)
  6. Signed Affidavit to Use the Surname of the Father (if parents are not married)
  7. Signed Joint Affidavit of Two Disinterested Persons (if parents are not married)
  8. 1 photocopy of Valid ID (preferably QCitizen ID) or Cedula

Additional requirements:

For 6 years old and below, any of the following:

  1. 1 photocopy of the immunization chart (baby book)
  2. 1 original and 1 photocopy of medical certificate (if hospital born)

For 7 to 17 years old, 2 of the following:

  1. 1 photocopy of the immunization chart
  2. 1 photocopy of medical record
  3. 1 photocopy of the school record (Form 137)
  4. Notarized Affidavit of Abandonment (If applicable)

Reminder:

Bring the original copies of the required documents for verification

Process:

  1. Get the Certificate of Live Birth form at the City Civil Registry Department.
  2. Submit the fully accomplished Certificate of Live Birth and all necessary requirements to the Birth Division of the Civil Registry Department.
  3. Wait for the release of the registered Certificate of Live Birth after the mandatory posting period of 10 days. 

How To Avail Burial Assistance

Provision of burial assistance to the bereaved families or relatives to lessen their economic burden due to the death of one family member through a Guarantee Letter Referrals to accredited funeral homes within Quezon City.

Requirements:

  1. Certified True Copy of Death Certificate registered by the City Civil Registry Department
  2. Original and Notarized copy of the Funeral Contract
  3. Original Barangay Certificate of Indigency (named after the relative or representative of the deceased)
  4. 1 photocopy of any Valid ID of the deceased or relative / representative.

Process:

  1. Submit the necessary requirements to Social Services Development Department and wait for the interview.
  2. If qualified, a Certificate of Guarantee will be issued to the applicant as payment to the partner funeral parlor.
  3. if not qualified, Social Worker will issue a Social Case Study Report addressed to other agency that provides burial assistance.

For those who do not have a funeral parlor:

  1. Contact Social Services Development Department at 8703-6823, and provide the following information:
    1. Name of Deceased
    2. Place of Death
    3. Address of the Deceased
    4. Contact person and contact number
  2. Submit all necessary requirements to Social Services Development Department and wait for the interview.
  3. If qualified, a Certificate of Guarantee will be issued to the applicant as payment to the partner funeral parlor.

Gawad Parangal

Eleven exceptional QCitizens and institutions who brought honor and pride to the city and the nation received the prestigious 20th Manuel L. Quezon Gawad Parangal as part of the city’s 83rd Founding Anniversary. Mayor Joy Belmonte and Vice Mayor Gian Sotto led the awarding ceremony which was held at the EVM Convention Center on October 27, 2022.

National Women’s Month

The City organized events and activities for Women’s Month to actively protect and promote the rights of every Filipina. On March 7, Mayor Joy Belmonte opened the celebration with her State of the City’s Women Address which discussed the current status of women from all sectors.

Kasalang Bayan

In line with the celebration of Valentine’s Day and the Month of Love, Mayor Joy Belmonte officiated Kasalang Bayan of 6,223 indigent couples from 6 districts. The ceremonies were held almost every day from February 1 to February 27. Couples who have been living together, some for as long as four decades, were finally able to solemnize their union through the Kasalang Bayan, for free.  Mayor Joy also led the commitment ceremony of 222 LGBTQIA+ couples last February 12, 2022
