HON. MA. JOSEFINA BELMONTE
City Mayor

Enforces all laws and ordinances relative to the governance of the city and in the exercise of appropriate corporate powers provided for under Section 22 of the Local Government Code, implements all approved policies, programs, projects, services and activities of the city;

Initiates and maximizes the generation of sources and applies the same to the implementation of development plans, programs, objectives and priorities as provided for under Section 18 of the Local Government Code, particularly those resources and revenues programmed for agro-industrial development and countryside growth  and progress; and

Ensures the delivery of basic services and the provision of adequate facilities as provided for under Section 17 of the Local Government Code.

Submit requirements to the City Civil Registry Department,  Civic Center Building C, Quezon City Hall Compound

Burial Assistance

REQUIREMENTS

  1. Certified true copy of Death Certificate reflecting Quezon City address
  2. Duly notarized original copy of Funeral Contract
  3. Certificate of Indigency from the barangay
  4. Valid Government I.D. of Representative
  5. Valid Government I.D. of Deceased

PROCEDURE

  1. Contact the Social Services Development Department (SSDD) office and provide the following information of the deceased.
    1. Complete name of deceased
    2. Place of death
    3. Residential Address (Quezon City address)
    4. Contact person and contact number of representative

      Arrangement for assistance / pick-up of cadaver will be shouldered by SSDD.
  2.  Proceed to the SSDD office with the death certificate for the processing of Interment / cremation permit
  3. Cremation will be scheduled
  4. Complete requirements should be submitted to SSDD
