How do I register for Vaccination?

If you have a QC-eServices Account: Go to qceservices@quezoncity.gov.ph andlog in to your QC-eServices Account and Click on “QC Vax Easy” from the list of services. Your details will automatically be pre-filled into the Registration Form. Click “Ok” to confirm.

OR

Go to https://qceservices.quezoncity.gov.ph/qcvaxeasy and click “Register for a Vaccination” and enter your QCeServices account e-mail address.

If you do NOT have a QC-eServices Account: Go to https://qceservices.quezoncity.gov.ph/qcvaxeasy and click “Register for A Vaccination”. Click “Continue without a QCeServices Account” fill out the “QC Vax Easy Registration” and click Submit and “Yes”

How can I edit information on my QC Vax Easy Registration?

If you used your QC-eServices Account to register for QC Vax Easy, you can simply go to qceservices.quezoncity.gov.ph, log in to your QC-eServices account, navigate to the QC Vax Easy portal and edit and resubmit your information. After updating, your QC Vax Easy Registration Information will automatically be updated.

If you do NOT have a QC-eServices Account: Go to https://qceservices.quezoncity.gov.ph/qcvaxeasy and click “Manage Your Vax Easy Registration”. Input your Reference No., Last Name, and Date of Birth to access your account. Click “View Registration” To edit, click “Edit Information”. You will be able to edit your mobile number and priority group only. To edit other information, please call our helpdesk at 122.

Please note that QC Vax Easy registrations are non-transferable to other people once created.

If you do not have your reference number, please call hotline 122 or email qcvaxeasy@quezoncity.gov.ph for your concerns.

After submitting my registration on QC Vax Easy, how will I get my vaccination schedule?

You will receive an SMS/text message on your registered mobile number OR you can choose to book your own vaccination appointment.

How do I schedule a COVID-19 vaccination appointment?

Step 1: If you have a QC-eServices Account: Go to qceservices@quezoncity.gov.ph andlog in to your QC-eServices Account and Click on “QC Vax Easy” from the list of services. 

If you do NOT have a QC-eServices Account, go to https://qceservices.quezoncity.gov.ph/qcvaxeasy and click “Manage Your Vax Easy Registration”

Step 2: Click “Book Vaccination”

Step 3: Click on the Calendar Button/Icon. The Calendar will pop-up. Choose your preferred vaccination date.

Step 4: Chose your preferred vaccination site from the list of sites for your chosen date by clicking on the circle icon beside the name of the Vaccination Site.

Step 5: Under Time, click on the Calendar Button/Icon and choose your preferred time. Click “Submit” to submit your preferred vaccination schedule.

Step 6: Click “Submit” to confirm your vaccination appointment. Do not close your window until you see a message that your vaccination schedule has been successfully booked. Please take note of this information. If you want to receive an email confirmation of your schedule, please provide your email when asked.

How do I RESCHEDULE a COVID-19 vaccination appointment?

Step 1: If you have a QC-eServices Account: Go to qceservices@quezoncity.gov.ph andlog in to your QC-eServices Account and Click on “QC Vax Easy” from the list of services. 

If you do NOT have a QC-eServices Account, go to https://qceservices.quezoncity.gov.ph/qcvaxeasy and click “Manage Your Vax Easy Registration”

Step 2: Click “Reschedule your vaccination”

Step 3: Click on the Calendar Button/Icon. The Calendar will pop-up. Choose your preferred vaccination date.

Step 4: Chose your preferred vaccination site from the list of sites for your chosen date by clicking on the circle icon beside the name of the Vaccination Site.

Step 5: Under Time, click on the Calendar Button/Icon and choose your preferred time. Click “Submit” to submit your preferred vaccination schedule.

Please take note of this information. You will receive an email confirmation of your vaccination schedule if you had previously provided your email address.

How do I CANCEL a COVID-19 vaccination appointment?

Step 1: If you have a QC-eServices Account: Go to qceservices@quezoncity.gov.ph andlog in to your QC-eServices Account and Click on “QC Vax Easy” from the list of services. 

If you do NOT have a QC-eServices Account, go to https://qceservices.quezoncity.gov.ph/qcvaxeasy and click “Manage Your Vax Easy Registration”

Step 2: Click “Cancel your vaccination”

Step 3: As your vaccination schedule appear, scroll down and click “Cancel Vaccination” Once your vaccine schedule has been cancelled, you will lose your slot permanently.

If you want to book another vaccine schedule, you need to click on the “Book Vaccination” button again though the QC Vax Easy home page.

Can I book my 2nd dose schedule?

No, the QC Vax Easy is only for 1st dose scheduling. Follow the schedule written on your vaccination card. If you missed your 2nd dose schedule, contact the District Health Officer.

How can I request for a new schedule if I cannot make it to my assigned 2nd dose schedule?

Please contact the City Health Department. Please be ready to provide the details of your previously scheduled 2nd  dose vaccination.

Who is eligible to register for QC VAX Easy?

Individuals who live or work in Quezon City. Please be sure to provide either a Quezon City residence and/or a Quezon City place of work in when you register for QC Vax Easy.

Can I register for a COVID-19 Vaccine even if I do not belong to the priority group currently being vaccinated?

Yes, you may register ahead. Note that you will only receive a notification once you have a scheduled appointment. Vaccination schedules are made when the priority group you belong in is already approved for vaccination.

Can I register multiple times?

No, the system bars people from registering multiple times. If you miss your vaccination schedule, you can request another schedule by resubmitting your registration.

Will my personal information remain confidential?

Data entered in QC Easy Vax will be kept confidential and used only for vaccination purposes. Moreover, only authorized personnel will be given access to your personal information.

How long do I have to wait for a text confirmation of my COVID-19 Vaccine through QC Vax Easy?

If you choose not to book your own appointment, city-scheduled appointments are made based on the several factors:

  1. Vaccine supply and availability.
  2. Priority Groups currently approved for vaccination.
  3. Date and Time of registration (First Come, First Served)

Can another person take my appointment if I cannot make it?

No. Vaccination schedules are non-transferrable and are scheduled specifically for registered individuals only.

What should I bring on the day of my scheduled vaccination?

a. Valid ID (Proof of residency or employment in Quezon City)

b. Confirmed Schedule. For QC Vax Easy: Text Message with Confirmed Schedule

c. Proof that you belong to the priority group being vaccinated.

    For persons with Co-morbidities or A3 (bring any of the following):

  • Medical Certificate issued within the past 18 months
  • Prescription of medicines for the past 6 months
  • Hospital Records (discharge summary and medical records)
  • Surgical records and pathology reports

   For Essential Workers or A4 (bring any of the following):

  • Company ID
  • Contract or Permit
  • Certificate of Eligibility (employment, deployment, assignment)
  • Other documents such as: business permit or community tax certificate.

Will I be informed of the vaccine brand?

No. As per DOH/IATF guidelines we cannot disclose which vaccine brand will be allocated. Once you show up to the vaccination site you will be informed of the brand of the vaccine.

Will I get any documentation that I am vaccinated?

Yes. A COVID-19 vaccination card would be provided after the first dose. The card will indicate you have been vaccinated, either with one or two doses. Your vaccination information will be forwarded to the Department of Health for use in the national vaccine passport program.

Do I need to register for my second dose?

No. A vaccination card / record shall be provided during your first dose. Follow the second dose schedule indicated on your vaccination card. Return to the same venue and at the same time as your first dose.

I have already registered with my barangay but have not been scheduled for my COVID-19 vaccine. Do I need to register in QC Vax Easy?

Yes, you may also register through QC Vax Easy. It is also possible that your Barangay have registered you on QC Vax Easy. If you do not have your reference number, please call hotline 122 or email qcvaxeasy@quezoncity.gov.ph for your concerns.

Will my eZConsult account be updated when I get my vaccination through QC Vax Easy?

No. QC Vax Easy is an alternative registration system and is not tied into EZConsult.
