QUALIFICATIONS:
- Ages 59 years old and below
- Monthly income of PHP 10,000 and below
- Not yet a member of PhilHealth
- Belongs to any of the following sectors:
- Barangay Volunteers and staff like street sweepers, BPSO, and Barangay Health Workers
- Day Care Parents
- Solo Parents
- Tricycle drivers
- Informal workers like construction workers, vendors, etc.
- Pregnant
- Abandoned/ foundling children
- With a chronic disease and needing continuous treatment like dialysis and chemotherapy
REQUIREMENTS:
- 1 original and 1 photocopy of the Barangay Certificate of Indigency
- 2 photocopies of the valid government-issued ID of the applicant (Preferably QC ID)
- 2 photocopies of the Certificate of Live Birth of the applicant from the Philippine Statistics Authority, if no valid ID
- 1 original and 2 photocopies of Medical Abstract or Medical Certificate from QC Health Department or Private Hospital (for walk-in and in-patient only)
- 1 original and 1 photocopy of the filled-up PhilHealth Member Registration Form (PMRF). The PMRF can be downloaded from the website of PhilHealth (www.philhealth.gov.ph)
PROCESS FOR WALK-IN CLIENTS IN INSTANCES SUCH AS WHEN CONFINED OR UNDERGOING DIALYSIS AND IN EMERGENCY CASES:
- The patient or his/her representative shall go to SSDD to process the application for PhilHealth and submit the requirements.
- Interview and assessment will be conducted by SSDD to ascertain the applicants’ eligibility to avail of the program.
- Wait for the call of SSDD staff within 3 working days to get the Certification to avail of PhilHealth benefits at the Department of Health.
- Bring the Certification and other requirements to your nearest PhilHealth Office to process the enrollment.
PROCESS FOR REFERRALS FOR BARANGAY, HEALTH CENTERS, AND OTHER ORGANIZATIONS OR GROUPS:
- Submit the requirements and the master list following the template that can be downloaded at the Official Website of Quezon City Government and the PhilHealth Member Registration Form (PMRF) that can be downloaded at the website of PhilHealth (www.philhealth.gov.ph).
- Wait for the checking of the submitted master list and requirements to be conducted by SSDD staff. The referring party will be immediately notified of the lacking documents for compliance in order to transmit the list of enrollees to PhilHealth within a week.
- Once the PhilHealth IDs are issued, SSDD with the help of the Barangays or officers of a group will inform the beneficiary via call or personal visit at their respective addresses of the schedule and venue of the distribution of IDs.
