Dalhin lamang ang requirements:

• Government-issued / Valid Identification Card For the representative of the document owner:
• Authorization letter or Special Power of Attorney from the document owner (original copy).
• Photocopy of Government Issued ID of document owner and representative with signature of the document owner.

Ipasa ito sa 6/F CCRD, District 3 Action Office, No. 25 Calderon Street, Barangay Marilag, Quezon City. Bukas ang kanilang opisina tuwing Lunes hanggang Huwebes, mula 8:00 AM – 5:00 PM at mula 8:00 AM – 3:00 PM tuwing Biyernes.
