STATEMENT OF THE QUEZON CITY GOVERNMENT

The Quezon City government has received some reports regarding discrepancies in the recording of vaccine administration in one of our sites.

While this was the first time it happened, we consider this as something that needs to be seriously and immediately looked into.

We are constantly working with our third-party partner, eZConsult, about the matter. Based on the preliminary probe, the issues identified are a result of human encoding error.

We sincerely apologize for any confusion this may have caused some of our vaccine patients. Rest assured that the local government and eZConsult will address the matter to prevent its reoccurence.

A handful of QCitizens also reported the discrepancy of vaccine brands reflected in-app and their vaccination card.

We want to assure everyone that we are working hard to reconcile such discrepancies. Our advice is to refer to your vaccination card for the correct brand of vaccine and the date of your second dose.

QC residents who have concerns regarding their vaccination details can also e-mail EZConsult via support.ezconsult@interpharmasolutions.com for clarification.

Despite this incident, the city government assures that the eZconsult website is the fastest, safest, and most reliable way for a resident to book a vaccination schedule.
