QUALIFICATION FOR APPLICATION:
- Available to Quezon City residents who are queer couples, or if at least one partner resides in Quezon City
REQUIREMENTS FOR THE ORIENTATION:
- Three (3) photocopies of QC ID (for QC residents) or three (3) photocopies of valid ID (for non-QC residents)
ONLINE APPLICATION:
- Log in to QC E-Services with your registered email.
- Choose ‘QC Right to Care Card’ from the list of services and click ‘Apply for Right to Care Card.’
- Input your personal information along with your partner’s personal details and click ‘save application.’
- You may check the status of your application on your eServices dashboard and set an appointment for the Right to Care Card Orientation. Check your email for confirmation of your registration.
- Attend the orientation on the date indicated in your registration. QC residents must bring a QC ID while non-residents must bring one valid ID.
- Following the orientation, you and your partner will be requested to sign a Special Power of Attorney (SPA) in the presence of the legal counsel.
NOTE: Expect to be notified by the Gender and Development Council Office (GAD) regarding the verification and status of your RTC application.
MANUAL APPLICATION:
- Visit the Gender and Development (GAD) Council Office located on the 6th floor of the High Rise Building inside the Quezon City Hall Compound.
- Once you have already applied for a Right to Care card, select your preferred schedule of orientation.
- Attend the orientation and bring either a QC ID (for QC residents) or one valid ID (for non-residents).
- Following the orientation, you and your partner will be requested to sign a Special Power of Attorney (SPA) in the presence of the legal counsel.
- You will be notified regarding the availability of your notarized e-copies of SPA and ID and informed of the status of your RTC application.
