  • 31 days old to 6 years old
    • Accomplished Form 102 (4 pcs)
    • Photocopy of Medical Certificate or Baptismal Certificate
    • Photocopy of Valid ID or Cedula
    • Photocopy of Marriage Contract (PSA Copy)
  • 7 years old to 17 years old
    • Accomplished Form 102 (4 pcs)
    • Photocopy of Medical Certificate or Baptismal Certificate
    • Accomplished Form 137
    • Photocopy of Valid ID or Cedula
    • PSA Negative Result

REQUIREMENTS FOR APPLICANTS WITH UNMARRIED PARENTS:

  • 31 days old to 6 years old
    • Accomplished Form 102 (4 copies)
    • Photocopy of Medical Certificate or Baptismal Certificate
    • Photocopy of Valid ID or Cedula
    • Affidavit to Use the Surname of the Father (signed by mother)
    • Affidavit of two witnesses
  • 7 years old to 17 years old
    • Accomplished Form 102 (4 copies)
    • Photocopy of Medical Certificate of Baptismal Certificate
    • Accomplished Form 137
    • Photocopy of Vallid ID or Cedula
    • PSA Negative Result
    • Affidavit to Use the Surname of the Father (signed by mother)
    • Sworn Attestation
    • Affidavit of two witnesses
