Quezon City Department or Office Involved:
OFFICE FOR THE SENIOR CITIZENS’ AFFAIRS 
SOCIAL SERVICES DEVELOPMENT DEPARTMENT

REQUIREMENTS IF BENEFICIARY:

  • QC ID (Physical Card)
  • Two (2) Photocopies of QC ID (back-to-back) with three (3) original signatures of applicant and contact number 

REQUIREMENTS IF AUTHORIZED REPRESENTATIVE:

  • Original QC ID of Beneficiary and Original Government Issued ID of Representative
  • Two (2) photocopies of ID (back-to-back) 
  • Beneficiary: Client with three (3) original signatures/thumbmark 
  • Authorized Representative: Printed Name and Signature
  • Authorization Letter with Signature or Thumbmark of the Beneficiary
  • Indicate the relationship of the Representative to the Beneficiary.
  • Indicate the purpose: To claim the social welfare assistance (SWA).
  • Indicate why the beneficiary can’t personally go to the venue.
  • Picture of Beneficiary together with Authorized Representative (print two (2) copies) holding recent newspaper with visible date (dated at least 1 week prior to the schedule of pay-out) or Calendar in a present month
  • Barangay Certification: If not immediate family (case-to-case basis)

NOTE: Immediate family includes Spouse, Parents, Siblings, Children.

REQUIREMENTS IF BENEFICIARY IS DECEASED:

  • QCID of the Deceased and one (1) Photocopy
  • Original and one (1) Photocopy of Death Certificate (include three (3) signatures) with registry number or Original Certified True Copy with  registry number from Local Civil Registry
  • Government-issued ID of Claimant and three (3) Photocopies with three (3) signatures
  • Original ID of the Deceased with Photocopy (Original ID must be surrendered to OSCA)
  • Barangay Certification (Certifying the relationship of Beneficiary and Claimant)

PROCESS:

  1. Check the Official Facebook page of the Office for the Senior Citizens Affairs in Quezon City for the payout schedule in your district or the Official page of Quezon City Government for other options to claim the financial assistance.
  2. Bring the requirements with you at the venue on the day of the payout.
