REQUIREMENTS:
- Certified copy of Certificate of Death
- Original Copy of Transfer or Entrance Permit (if death occurred in another city/municipality)
- Affidavit of Undertaking (if the remains will be interred in Bagbag Public Cemetery or Novaliches Public Cemetery)
- Burial Form from Quezon City Health Department
- Valid Identification Cards (IDs) of the informant/family/relative/authorized person
PROCESS:
- Submit the documentary requirements to the receiving clerk for evaluation at the City Civil Registry Department (CCRD) office located at door 2 of the Civic Center Building C at the Quezon City Hall Compound.
- Secure the order of payment.
- Hand the order of payment to the cashier at the City Treasurer’s Office (CTO) to settle the payment fee and get the official receipt. Here are the following fees:
- Burial Permit – 100.00 PHP
- Entrance/ Transfer Permit – PHP 100.00 (If the interment is in Bagbag / Novaliches Public Cemetery)
- Niche for child: PHP 750.00
- Niche for adult: PHP 1500.00
- Return the official receipt to the receiving clerk in Window 6 or 7 at the CCRD office, then receive the burial permit.
