A QCitizen ID allows you to apply for local government services online and avail the promos and discounts in selected establishments.
REQUIREMENTS:
- For Lost QCID Card
- Affidavit of Loss
- Photocopy of Valid ID
- For Damaged Card
- Signed Attestation Letter or Incident report
- Surrender of old QCID card
- For Change of Residential Address
- Signed Request Letter
- Proof of Residency
- Surrender of old QCID card
- For Change of Email Address (QC E-Services)
- Signed Request Letter
- QCID card must be shown
- For Correction of Other Details
- Signed Request Letter
- Surrender of old QCID card
SUPPORTING DOCUMENT/S OF CORRECTION:
- PSA Birth Certificate (for Change of Name or Birthdate)
- Valid ID with new signature or Affidavit of Discrepancy (for Correction of Signature)
- Medical Certificate or Affidavit of Discrepancy (for Correction of Blood Type)
- Marriage Certificate/Divorce or Annulment Decree/Death Certificate of Spouse (for Change of Civil Status)
PROCESS:
- Proceed to the QCID Office located on the 2nd floor of the High Rise Building at Quezon City Hall or at the District Action Office near you and submit the required documents.
- Wait for the requirements to be verified and for the Order of Payment to be issued.
- Proceed to the Miscellaneous Fees Section of the City Treasurer’s Office. Present the Order of Payment issued and pay the PHP 100.00 replacement fee. An official receipt will be issued after payment.
Exemptions from Fees:
- New Applicants of QCID
- Indigent Applicant (Must present a Certificate of Indigency)
- Residents of a specific area or barangay that has been affected by a disaster
- Present the official receipt along with the required documents to the QCID personnel to claim the reprinted QCID physical card at the releasing window.
