REQUIREMENTS:
- Exhumation Letter to be secured from Quezon City Health Department (QCHD)
- Certified copy of Certificate of Death
- Valid Identification Cards (IDs)
PROCESS:
1. Submit the Exhumation Letter, together with the Certified Copy of Certificate of Death to the receiving clerk at the City Civil Registry Department (CCRD) Office located at Door 2 of Civic Center Building C at the City Hall Compound.
2. Secure the Order of Payment from the City Treasurer’s Office located at Annex Building.
3. Submit the Order of Payment to the cashier and secure the Official Receipt. Here are the corresponding fees:
- Exhumation of Cadaver Fee – PHP 100.00
- Removal of Cadaver Fee – PHP 100.00
4. Return the Official Receipt to Counter 6 or 7, and secure the Exhumation Permit, which is in the form of an Official Receipt.
