ELIGIBILITY:
- Resident of Quezon City who owns QCID
REQUIREMENTS
For Medicines & Medical Supplies:
- Original or Certified True Copy of Medical Certificate (for outpatient) or Clinical Abstract (for confined patient) with complete name, signature and license number of the attending doctor (with 3 months validity or issued not less than 3 months)
- Latest and original prescription of the patient with complete name, signature and license number of the attending doctor (with 3 months validity or issued not less than 3 months)
- Original or Certified True Copy of Barangay Certificate of Indigency of the patient (with 3 months to 6 months validity)
- One (1) photocopy of any Government Issued Valid ID (preferably QCitizen ID) of patient or his/her representative or claimant
For Medical Laboratories:
- Original or Certified True Copy of Medical Certificate (for outpatient) or Clinical Abstract (for confined patient) with complete name, signature and license number of the attending doctor (with 3 months validity or issued not less than 3 months)
- Latest and original laboratory request with price quotation and complete name, signature and license number of the attending doctor (with 3 months validity or issued not less than 3 months)
- Original or Certified True Copy of Barangay Certificate of Indigency of the patient (with 3 months to 6 months validity)
- One (1) photocopy of any Government Issued Valid ID (preferably QCitizen ID) of patient or representative or claimant
If Authorized Representative:
- QCID or Valid ID
- Authorization Letter stating the relationship with the patient
ONSITE PROCESS:
- Visit the Information Desk at the District Action Center near you.
- Present the requirements.
- Undergo an intake interview, where your application will be assessed.
- You may receive Mercury Drug Gift Certificate or medical bill assistance for healthcare facilities with existing memorandum of agreement such as the Quezon City General Hospital, Lung Center of the Philippines, National Children’s Hospital, National Kidney and Transplant Institute, Heart Center of the Philippines, East Avenue Medical Center, Philippine Children’s Medical Center, Quirino Memorial Medical Center, and St. Luke’s Medical Center.
ONLINE PROCESS:
- Log in or create an account on the QC E-Services portal using the registered email address of the patient or representative.
- Look for the SSDD (Social Services Development Department) icon and click “Financial Assistance – Medical Assistance Program”.
- Click “Apply for a Service” and choose “Medical Assistance Program”.
- Set an appointment schedule and click “submit”.
- Wait for the confirmation of your appointment for assessment and interview.
- Bring the screenshot or the copy of the appointment and the complete requirements during the day of your schedule.
- You may receive Mercury Drug Gift Certificate or medical bill assistance for healthcare facilities with existing memorandum of agreement such as the Quezon City General Hospital, Lung Center of the Philippines, National Children’s Hospital, National Kidney and Transplant Institute, Heart Center of the Philippines, East Avenue Medical Center, Philippine Children’s Medical Center, Quirino Memorial Medical Center, and St. Luke’s Medical Center.
