BUSINESS PERMIT AND LICENSING DEPARTMENT
CITY TREASURER’S OFFICE
All business permit applications forms have now been consolidated in one Unified Application Form for the convenience of applicants.
REQUIREMENTS:
- Proof of Business Registration
- Sole Proprietor – DTI Registrationn
- Business name on DTI must match the business name on application
- DTI must be valid on the date of application
- Coverage level of DTI (Barangay, City/Municipality, Regional, National) must cover the location of the business
- Owner’s name on DTI must match the business owner’s name on application
- Corporation/Partnership/OPC – SEC Registration
- Business/Company/Corporate Name on SEC Registration must be the same on application
- Line of business/Business activity on application must be included on the purpose of SEC Registration
- For SEC Registrations that are 5 years older or more, it must be uploaded with the most recent GIS of the corporation (except OPC)
- If the principal address indicated on SEC registration is the same with the business address on application, total declared capital investment should be the same or more with the paid-up capital indicated on the SEC registration
- President’s name must be stated on the SEC Registration or GIS
- Cooperatives – CDA Registration
- Cooperatives’ Name on CDA Registration must be the same on application
- Line of business/Business activity on application must be included on the purpose of CDA Registration
- President/Chairman’s name must be stated on the CDA Registration
2. Proof of Authority to use Business Address
- If property is owned – Proof of ownership such as but not limited to the following: TCT, RPT Receipt, and Tax Declaration.
- Proof of ownership under the name of the business owner (for sole proprietor) or one of the incorporators/partners/members (for corporation, partnership, and cooperatives)
- For proof of ownership that is under a different name, require document(s) that will show connection between the property owner (as indicated on the proof of ownership) and the business owner. This document must also show that the property owner acknowledges the operation of business on their property
- Business area must be less than or the same with the area indicated on Tax Declaration
- If property is rented/leased – Contract or any Agreement document such as but not limited to the following: Contract of Lease, Memorandum of Agreement, Award Notice, or Other document(s) that shows acknowledgement and/or approval to utilize the premises for business/commercial purposes
- Document must be duly notarized and should include the ID numbers of the parties involved
- Complete address must be indicated and must be the same with the business address on application
- Document must show that the business owner is allowed to utilize the premises
- Contract/Agreement must be valid upon application
- If property is government owned – Affidavit of Voluntary Vacate
- Affidavit must state that the premise(s) is owned by the government
- Affidavit must be signed and executed by the business owner
- Affidavit must be duly notarized and should include the ID details of the business owner
- Others (Consent, Usufruct, etc.)
- Document must show the complete address of the premises and must be the same with the business address on application. It must also state/show acknowledgement or approval of the property owner to utilize their property by the business owner for business or commercial purposes
- Document must be duly notarized and should include the ID numbers of the parties involved
- Document must be accompanied by any valid government issued ID of the property owner
- Document must be duly signed and executed by the property owner
- For property owned by entities with artificial identity (corporation, partnership, etc.), signatory on the document must be authorized to execute such document. Supporting document(s) showing this authority such as but not limited to: SEC Registration, GIS, Board Resolution, etc. must also be provided
3. Authorization Form (for application that will be processed by an authorized representative, follow the steps below):
- Download and complete the form with all required information.
- Ensure the form is signed by the owner of the application.
- Scan the signed form and the Valid IDs of the owner and the representative.
- Proceed with the Short Term or Special Permit application and upload the scanned file.
ONLINE APPLICATION:
- Log on to the QC E-Services portal.
- Among the list of services, select ‘Business One-Stop Shop’.
- Select ‘Click Application Online’.
- Select ‘Amendment’.
- Input ‘Mayor’s Permit Number’ and Business Tax official receipt for the current year.
- Submit requirements, depending on the information to be amended.
- Wait for an order of payment via the registered email address. Pay the corresponding fee at the City Treasurer’s Office located at the Quezon City Hall Compound.
- Submit proof of payment or scanned copy of the Official Receipt via email at BPLD@quezoncity.gov.ph.
- Once the payment has been validated and the application approved, an e-copy of the business permit will be sent to the registered email address.
- Wait for notification via email to claim the renewed permit in-person, or have it delivered via a courier of choice.
