QUALIFICATION:
- Filipino resident of Quezon City who is sixty (60) years of age or older
NOTE: This may apply to seniors with “dual citizenship status” if he/she can prove his/her Filipino Citizen status and has resided in this city for at least six (6) months.
REQUIREMENTS:
- Fully Accomplished QCitizen ID Online Form or Request for Sectoral Status Change (if already a holder of regular resident/non-senior QC ID) in QC E-Services
- Printed Application Form (for onsite assisted encoding at the Office for Senior Citizens’ Affairs)
- 1 original and 1 photocopy of valid ID with applicant’s birth date and address (if no QCitizen ID)
- 1 original and 1 photocopy of the Birth Certificate and Barangay Certificate of Residency (if there is no QC ID or the birthdate and address are different from the Valid ID)
- 1 original and 1 photocopy of the Marriage Certificate (if the applicant is a married woman whose last name is different from the Valid ID)
- Latest digital or ID photo (2×2) – Colored with white background
ADDITIONAL REQUIREMENTS FOR REPRESENTATIVES:
- Application form for OSCA ID
- Authorization Letter signed by the applicant
- 1 original and 1 photocopy of the valid ID of the representative (Preferably QC ID)
ONLINE APPLICATION PROCESS:
- Create a profile and account with QC E-Services.
- From the QC E-Services menu, select QC ID e-Application.
- Fill in all required information or data. If you already have a resident/non-senior QC ID, select Request for Sectoral Status Change.
- Wait for the email confirmation once your application has been approved.
- Download the QCitizen App on the smartphone to get a digital copy of your QC ID.
- Wait for the physical ID to be delivered to your registered address.
PROCESS FOR OFFLINE/WALK-IN APPLICATION:
- Go to the Official website of Quezon City and download the S.C. QC ID Application Form or obtain a form from OSCA.
- Submit the Fully Accomplished Senior Citizen QC ID Application Form and all required documents for OSCA verification.
- Once approved, the ID will be sent to the senior citizen’s home or can be picked up at OSCA.
