    • Click Health Certificate and Sanitary Permit
    • Accomplish the Online Health Certificate Application Form
    • Attach all the necessary documentary requirements:
      • ID Photo (2X2 ID Picture with white background)
      • Proof of valid HIV Seminar from authorized institutions
      • For applicants below 18 years old:
        • Original copy of birth certificate
        • Written Parental/ Guardian consent
        • Original Valid ID of Parent/Guardian giving consent
    • 1.4 Monitor the status of the application (View Progress or Individual History Status)
  2. Print Order of Payment (for Health Certificate fees)
    • WALK-IN PAYMENT
      • Pay corresponding fees to the City Treasurer’s Office Cashier or any CTO branch/satellite offices:
        • Health certificate processing fee: PHP 75
        • HIV Seminar fee: PHP 50
        • Laboratory fees:
          • Sputum Exam: PHP 110
          • Stool Exam: PHP 65
    • ONLINE PAYMENT
      • Transfer/ Deposit payment to Landbank of the Phils. Account:

      • Receive the photo of Official Receipt (OR) thru email.
      • Pick-up the original Official Receipt (OR) at the City Treasurer’s Office, 3rd Floor, Online Payment Group (present the valid ID and photo/digital copy of OR as reference). If pick-up by representative, present valid ID and authorization letter or email the name of representative
      • NOTE: Validation for online payment will take about two (2) or more working days depending on the number of days payment remittance was received.
    • Upload and submit Official Receipt
  3. Submit laboratory specimens (stool and sputum) to any QCHD In-house clinical laboratory (electronic medical examination results will be automatically uploaded in the system)
  4. Watch the video and answer the questions:
    • Wait for the seminar line which will be sent to your application once your payment receipt or Official Receipt is uploaded and submitted
    • Answer questionnaires in TEST KNOWLEDGE
    • Then click Submit
      • NOTE: If a valid HIV Seminar certificate was uploaded, the applicant will skip this step
  5. Generate a Digital Health Certificate and print it in the prescribed size (14 cm. x 9 cm.)

NEW APPLICANTS AVAILING EXTERNAL (ANY DOH LICENSED) CLINICAL LABORATORY SERVICES

REQUIREMENTS:

  • Specimens of applicant: Sputum AFB and Stool/Fecalysis
    • Stool examination (valid within three (3) months from date of examination)
    • Sputum AFB examination (valid within three months from date of examination) or Chest X-ray  (valid within six months from date of examination)
  • Official receipt of payment for health certificate and HIV seminar fees
  • HIV seminar (online or proof of valid HIV Seminar from authorized institutions) 
  • Additional Requirements for applicants below 18 years old:
    • Original copy of birth certificate
    • Written Parental/ Guardian consent
      • Original Valid ID of Parent/ Guardian giving consent

  3. Present Official Receipt and submit laboratory specimens (stool and sputum) to any QCHD In-house clinical laboratory (electronic medical examination results will be automatically uploaded in the system)  
  4. Once system is up, log-in to QC E-Services, and select Health Certificate & Sanitary Permit application services by using a device with updated browser
  5. Attach all the necessary documentary requirements:
    • ID Photo (2X2 ID Picture with white background)
    • Previous year health certificate/card or HIV seminar certificate (proof of valid HIV Seminar from authorized institutions
    • For applicants below 18 years old:
      • Original copy of birth certificate
      • Written Parental/ Guardian consent
      • Original Valid ID of Parent/ Guardian giving consent
