• A fully-accomplished surrender/pick-up form
• Government-issued ID

STEPS:

1. Submit Surrender/Pick-up form to the receiving clerk
2. Verification of Requirements
3. Pay Surrender/Pick-up Fee of P500.00
4. Wait for the scheduled pick-up of surrendered animals
