REQUIREMENTS:
- QC-Persons With Disability Identification
- Request letter addressed to Mayor through Head of QC-Persons with Disability Affairs Office
- Brgy. Indigency (Purpose: Assistive Devices Request)
- Cedula
- Supporting documents:
- Whole Body Picture & Medical Record (if Mobility Assistive Devices are requested)
- Hearing Assessment/ Audiogram (if Hearing Assistive Devices are requested)
PROCESS:
- Head to the Persons with Disability Affairs Office (PDAO) at the Quezon City Hall Compound and submit the pertinent requirements to a Community Based-Inclusive Development Staff for preliminary verification and evaluation.
- Undergo an intake interview with the assistance of a Community Based-Inclusive Development staff.
- Wait for your application to be evaluated and expect a call and/or text regarding the release of the requested assistive device.
- Receive the requested assistive device at PDAO and sign the Deed of Donation and Granting of Assistive Device.
