REQUIREMENTS:

• NBI or Police Clearance
• Health Certificate Number (if applicable) for applicants working in the following industries:
  • Commissaries/Eating Establishments
  • Hotels, Condominiums, and Other Places of Dwelling
  • Massage Clinics/Health Clubs
  • Night Clubs/Bars

NOTE: For applicants not working in the above-mentioned industries, the health certificate serves as a post-permit compliance.

For First-Time Job Seeker:

• PESO Certificate

For Foreign Applicant:

• Alien Employment Permit (AEP, Department of Labor and Employment)

For Minor Applicant:

• Parental Consent
• Valid ID of Legal Guardian
• Birth Certificate of Applicant

ON-PREMISE APPLICATION PROCESS:

1. Submit all required documents, including the application form, to the Occupational Permits Section of the Business Permits and Licensing Department office located at the Ground Floor of Civic Center Building A at Quezon City Hall.
2. Proceed to the cashier at the ground floor of the same building, where you will present the original order of payment and pay the corresponding fee ranging from P20.00 – P170.00. Claim the official receipt afterwards.
3. Present the official receipt to the Occupational Permits Section Personnel at the Business Permits and Licensing Department for validation.

NOTE: A digital occupation permit will be sent to the applicant’s email address.

ONLINE APPLICATION:

1. For an alternative and more convenient application process, you may log in to QC E-Services and select ‘Occupational (Work) Permit‘ from the list of services on the dashboard.
2. Complete the online application form and upload all necessary documents for evaluation.
3. Once you have received the order of payment, you may pay the occupational permit fee through QC Pay Easy with payment methods such as GCASH and PayGate.
