A Birth Certificate is a vital document that establishes the birth of a child. It is an official record that includes the birthday, place of birth, name of parents, and other important information that can be used for any official transaction.
REGISTRATION OF BIRTH CERTIFICATE (REGULAR AND TIMELY)
REQUIREMENTS:
- Duly Accomplished Certificate of Live Birth form (4 copies)
- For Marital Child (children born at the time of marriage of the parents):
- Marriage Certificate of Parents
- Copy of valid IDs of parents (Passport if one or both parents is a foreigner)
- For Non-Marital Child (children from outside of marriage of the parents):
- Signed and notarized Affidavit of Acknowledgment/Admission of Paternity (found at the back of the Certificate of Live Birth, if the child is acknowledged by the father
- Notarized Affidavit to Use the Surname of the Father (AUSF) (If surname of the father will be used) executed by:
- Mother; or
- Father (deceased mother or in case of abandonment)
- Copy of valid IDs of parents (Passport if one or both parents is a foreigner, with latest date of arrival)
ONLINE APPLICATION:
- Log in to QC E-Services Account.
- Click “Civil Registry Online Services”.
- Under the Birth Services section, click “Birth Registration”.
- Fill in all the required fields with the correct information about the child and the circumstances of birth.
- Upload the digital copies of the required documents to the assigned folder.
- Once the documentary requirements are deemed complete, an email notification will be received stating that the original documents must be submitted.
- Proceed to the City Civil Registry Department Office to submit original documents.
- Receive personal copy of registered Certificate of Live birth.
MANUAL/ ONSITE APPLICATION:
- Fill out the Certificate of Live Birth form and submit it to the City Civil Registry Department together with other requirements.
- The receiving clerk will check if the requirements are complete and will assign a registry number to the client.
- The clerk will release the document to the registrant.
Note: This service is free of charge.
REGISTRATION OF BIRTH CERTIFICATE (LATE FILING)
Registration of the birth of a child in Quezon City beyond 30 days from the day of birth will be subject to the delayed birth registration process.
REQUIREMENTS:
- Accomplished Certificate of Live Birth for late registration form (4 copies)
- Signed and Notarized Affidavit for Delayed Registration at the back of the Certificate of Live Birth form
- Negative Certification of Birth from PSA or National Archives of the Philippines, if applicable
- Certificate of No Record (CNR) from City Civil Registry Department
- Any two of the following documents which show the name, date and place of birth of the child, name of the mother and father (if acknowledged):
- Baptismal Certificate (1 original, 1 photocopy)
- School Record (Form 137) or Report Card(Form 138)
- Income Tax Return of Parents
- Insurance Policy
- Medical Records
- Barangay Certification
- Member’s Data Record
- Member’s Data Form
- SSS E-1 Form
- Voter’s Certification (with Voter’s RegistrationRecord, if the certification has no information on the place of birth)
- For Marital Child:
- Parents’ Certificate of Marriage
- Passport (if one or both parents is a foreigner)
- For Non-Marital Child:
- Signed and notarized Affidavit of Acknowledgment/Admission of Paternity (found at the back of the Certificate of Live Birth, if the child is acknowledged by the father;
- Notarized Affidavit to Use the Surname of the Father (AUSF) executed by:
- Mother or Father (deceased mother or in case of abandonment) for children below 7 years old
- Child if 7 to 17 years old, with Sworn Attestation of the mother or Father (deceased mother/ in case of abandonment)
- Child if 18 years old and above (majority age)
- Acceptable and recognized proof of acknowledgement by the father, if the child is born before August 3, 1988 and the father is deceased
- Joint Affidavit of Two Disinterested Persons / Witnesses
- Government issued ID or Cedula
- Passport (if one or both parents is a foreigner, with latest date of arrival)
ONLINE APPLICATION:
- Log in to QC E-Services Account.
- Click “Civil Registry Online Services”.
- Under the Birth Services section, click “Birth Registration”.
- Upload the digital copies of the required documents to the assigned folder.
- Fill in all the required fields with the correct information about the child and the circumstances of the birth.
- Once the documentary requirements are deemed complete, an email notification will be received stating that the original documents must be submitted.
- Proceed to the City Civil Registry Department Office to submit the original documents.
- Pay the corresponding fee of PHP 200 for the Late Registration at the City Treasurer’s Office.
- Once paid, the clerk will provide a document indicating the release date of the record.
MANUAL/ ONSITE APPLICATION:
- The City Civil Registry Department Receiving Clerk will request the client to secure a Certificate of No Record (CNR).
- Go to the City Treasurer’s Office cashier to pay the CNR fee of PHP 150.
- After obtaining the CNR, submit all the necessary requirements to the receiving clerk.
- The receiving clerk will then issue the client a Certificate of Live Birth form.
- After completely filling out the form, the clerk will issue an order of payment.
- Pay the Late Registration fee of PHP 200 to the City Treasurer’s Office cashier.
- Once paid, the clerk will provide a document indicating the release date of the record.
